Immunizations
In past years, thousands of Wisconsin children caught diseases such as measles, pertussis (whooping cough) and rubella, and many were left with severe disabilities. The Student Immunization Law was passed in order to keep these and other vaccine-preventable diseases from returning and harming the health of our children. The Student Immunization Law requires that all students through grade 12 meet a minimum number of required immunizations or select a waiver option prior to school entrance.
Below are forms that may be needed to provide the school with immunization records for your child. It is a parent's responsibility to provide all immunization records and make sure that their child is in compliance with the current Wisconsin Immunization Law.
Immunization records can be found using the Wisconsin Immunization Record website or the student's physician.
